When you see the form you want, select it and click the "Create" button to open a copy. Switch to the Developer tab and click on the "Design Mode" item to access form controls.

Create an HTML Form Click "File," then choose "New" and "Blank Page." A fresh HTML page appears. Select "Insert," then "Form." Click "Form" again. The form area becomes visible.
